interview

Completed the surgery of sinus maxillary and narrow bone with digital solution

I adopted it as I found this is exactly what I was looking for. To find a digital system for our hospital, we demonstrated several systems, but eventually we chose DIOnavi. solution.

Dr. Kang Jae-suk of Yedam dental hospital in Mokpo was worried about the efficiency of digital prosthesis manufacturing in 2013. He decided to adopt the digital system after visiting CK dental hospital in Ulsan which was considered as the first digital hospital in Korea at the time.
Yedam dental hospital was a large-scale dental hospital in the area which there were 50 dental units, about 10 dentists and about 60 dental staffs. The hospital didn’t have a laboratory itself, so prosthesis production was outsourced.
Three years after the digital introduction, most of the dental prosthesis is not produced directly in the hospital. The size of laboratory has been expended from one technician to 3 technicians. Unlike other hospital, the oral scanner is used in the laboratory as well as in the hospital. In the hospital, a digital system is built with 2ea Trios 3 of cart type and 1ea Trios 2 in the laboratory.

Digital dental hospital without hesitation
Of course, Yedam dental hospital does not do all the dental service digitally.
Several doctors also conduct analogue care each way, and some doctors focus on digital treatment like Dr. Kang.
For those patients who require bone graft due to severe bone defects, Dr. Kang also perform the treatment as part of analog protocol.
However, the hospital has been taking advantage of intra oral scanner data-enabled in the past three years.
As a result, he became convinced that the prosthetics could be made without a model. It is important to realize the accuracy of the prosthesis and the acceptability of patients.
Prosthetics accuracy is the key to digital systems
The most frequent use of digital in dental clinic is implant surgery using a surgical guide.
Dr. Kang as prosthetist noted that the problem of prosthesis fit can be caused by an error. Most of surgical guide systems on the market only consider the surgery stage, while DIOnavi. covers restoration part as well. Theoretically, there is a concern about fitness at the prosthetic lever if the deviation range is more than 3 degrees.
However, the deviation within 1 degree on the model makes significant large error in actual mouth in domestic and foreign guide system. The deviation of DIO which is 1.4 degrees is regarded as the most precise in the world with the smallest error. The related acritical will be announced soon by Prof. Choi Byung-ho.
Compatibility ‘good’ precision ‘good’
There are three main reasons why Dr. Kang chose DIOnavi.
First, DIO has chosen 3Shape to be compatible and widely used around the world, with many software programs ranging from prosthetics, implants, and orthodontics. The second is precision.
"It is often known that sleeves influence precision in guided surgery, but the precision depends on the drill system.
While many guide systems are focused on reducing the number of drills or simply helping a dentist place an implant on right position, DIO is a system that takes into consideration of process from fixture placement to prosthetics." he said.
The third reason is that DIOnavi. satisfies to cover wide range of surgery such as maxillary sinus and narrow bone surgery which other companies cannot provide the service. “It seems DIO is the fastest digital technology in dentistry.
I’v used several digital systems, and I think it’s similar to placement, but DIOnavi. is the closest to the idea of real flapless surgery thinking about prosthetics.” he said.
Prove objective competitiveness through patient response
What difference does the flapless concept have in the clinic?
Dr. Kang said, “What we objectively know is patient response. Once performing surgery, there is no pain trigger for patient. I think it’s possible to check whether the implant is properly placed in a narrow bone by CBCT because it’s flapless surgery.
Especially, in the edentulous case, dentist and patient often have difficulties, but flapless guided surgery make them relieve. However, he advised digital beginner to stick to the education program provided by each company. Without enough education, errors can occur and dentist can doubt the system itself.
Dr. Kang said that digital treatment reduces the inconvenience felt by the patient or the dentist and the fear of surgery, and is highly satisfactory after operation to dentist who opened their own clinic and hesitate to adopt digital. “We have prepared a comprehensive explanation based on the video of treatment procedure so that the patient can understand and choose the digital treatment, and the approval rate is more than 90%.” he said.
“Also, the age of digital treatment that patients choose is coming. Boldly challenge. Digital treatment is rather more interesting than conventional treatment. There is fun to try and apply. My team also enjoys the challenge of new system. It’s also possible to use digital treatment as a part of reimbursement of falling implant treatment fee. Yedam dental hospital has recently introduced 3D Printing System(DIO PROBO) to print out surgical guide directly in the hospital.
